What do I write on the blood pressure log?

Copy the date, time and displayed numbers onto a row. Systolic is the upper blood pressure number and diastolic is the lower one. The pressure columns show mmHg, and the pulse column shows beats per minute. Use the notes column for a short note you want to discuss at an appointment.

If you take several readings, keep them on separate rows rather than averaging them on this form. Follow the measurement and recording instructions given by your clinician. The American Heart Association provides a separate home-monitoring guide in the source link below.

How do I print a log with room to write?

Choose Letter, A4 or 11 x 17 and download the PDF. It uses a landscape layout, clear column headings and tall blank rows. The selected setting changes the title and row space; column headings use up to 24 pt. Print at Actual size so the cells keep their intended dimensions.

The weekly planner can hold appointment dates. Keep contact numbers on the phone book and other blank forms in large print lists.

Is the printable log saved online?

The blank PDF is generated in your browser. This tool has no fields for entering blood pressure measurements, and it does not upload a completed log. Store the handwritten pages with your own records and bring the relevant pages to an appointment.
